The Managed Care Organization (MCO) contracts of the Minnesota Department of Human Services (DHS) require all MCOs to send a quarterly report for all oral and written Grievances and Appeals, and all Denials, Terminations, and Reductions (DTR) of services to DHS.

When submitting quarterly data, MCOs may upload their data in a single file submission or through multiple files during the 30-day period following the end of a quarter. All DTRs that were issued within the quarter and all grievances and appeals that were resolved within the quarter are due by the 30th calendar day of each month following the end of a quarter. Data from a previous quarter can be uploaded by specifying the year and quarter to which the data belongs before submission.
